Request your health records
- Send a written request to intake@bgapsychology.com with the subject “Records request,” or call 512-368-8609 for help arranging a secure request.
- Include your name, safe contact information, the records and date range requested, and whether you want to inspect records or receive a copy. Tell us your preferred delivery method. Do not include diagnoses, identification documents, or other sensitive material in an initial unsecured email.
- If you are a representative, identify your role. We will explain how to verify identity and authority, provide any required authorization securely, and arrange access or delivery. Any lawful copying charges or limitations will be explained.
Requests are handled within applicable legal deadlines. Clinical records, test data, and protected test materials may be subject to different access rules. If access is restricted or denied, we will provide the explanation and review information required by law. You may also request a correction or confidential communications through the same contact.

To complain, visit BHEC's complaint instructions, complete its required complaint form, and submit it as directed to Enforcement@bhec.texas.gov or by mail to the address above. Follow the agency's directions for supporting documents. For services provided in another jurisdiction, its licensing authority may also be relevant.
File a consumer privacy complaint
You do not have to complain to the practice before contacting a regulator.
- Texas Attorney General: read the patient privacy information, then use the consumer complaint portal and select the appropriate privacy complaint. Describe the concern, relevant dates, and business involved. Follow the portal's directions and avoid including sensitive identifiers or unnecessary medical information.
- U.S. Department of Health and Human Services, Office for Civil Rights: for a concern covered by federal health privacy law, follow OCR's health-information privacy complaint instructions or call 800-368-1019.
